Diabetes Education

Our Diabetes Education Program is designed to provide our clients with the necessary knowledge and skills for successful diabetes self-management.  To insure that participants receive quality education, the program includes a diabetes educator and a dietitian.  An individual assessment is done at the first visit to help us understand how each person is currently managing their diabetes and identify areas for improvement.  Each person will then receive individual and/ or group instruction.  The topics that are taught include:

                - an overview of diabetes
                - what well controlled diabetes means
                - food and nutrition for better diabetes control
                - preventing long-term complications
                - staying active to better control blood sugar
                - medications for diabetes
                - treating excessively high or low blood sugar
                - realistic goal setting

A doctor's referral is necessary to participate in the program.  Upon completing the program, your doctor is informed of your progress.  A follow up session 1 to 3 months after completing the program is done to evaluate the progress of each participant toward the goals they set for managing diabetes.  Medicare and most insurances will pay for diabetes education.  For more information please call (304) 235-8528.
